What happens when a scenario fails in acceptance testing?

The result is recorded as failed together with a classified reason: product defect, missing configuration, undelivered development, bad test data, permission or environment blocker, training gap, or the scenario itself being written wrong. Without that distinction every failure lands in one bucket labelled "software bug" at the handover meeting, and the bill goes to the wrong side. We also record who signed off each result; results approved by the party that did the work are flagged as an independence signal.

What is a UAT scenario and why does it matter?
